An 11-year-old boy with leg length discrepancy (LLD) secondary to Legg-Calvé-Perthes disease underwent growth suppression using a tension-band plate (TBP) in the distal femur. At 13 years of age, an intercondylar longitudinal stress fracture of the femur was identified on radiographs before implant removal, despite successful LLD correction. Although 3D CT revealed significant asymmetry in the bone morphology, conventional radiography showed no apparent changes. The fracture healed uneventfully after implant removal./r/nThis case underscores the need for careful postoperative monitoring after TBP because stress fractures and subtle 3D morphological changes may occur without obvious radiographic signs.
