This case report details the rare presentation of a scaphoid waist fracture in a 27-year-old right-hand-dominant man, with an atypical displacement of the proximal fracture fragment into the flexor compartment of the forearm. Urgent open reduction and internal fixation using a Herbert screw were performed, resulting in fracture union and favorable wrist function at the 1-year follow-up./r/nThis case serves as a reminder that tailored, timely interventions for challenging scaphoid fractures can yield favorable long-term results, underscoring the significance of individualized approaches in fracture management.
