In this month’s Editor’s Pick, myAO is featuring the winning case of the myAO Spine competition. Now, we invite you to review the case that generated the most engagement from the myAO community, which was submitted by Mina Safwat Fekry from Egypt. This is a complex case of a 5-year-old male with C1–C2 instability and neurological deficits following a major trauma caused by a fall from height. The patient presented with myelopathy and left hemiplegia, managed initially with ATLS protocol. Imaging revealed C1–C2 instability with fracture at the skull base, requiring posterior C1–C2 fusion. Special considerations were taken due to the unique challenges of spinal immobilization in young children.
