This is a challenging case of a 58-year-old farmer presenting with a complex distal femur intra-articular fracture following a fall from a 3-4 foot height. The patient had pre-existing knee pain, osteoarthritis, and osteoporosis, complicating the management. Treatment included fixation and bone grafting through a medial para-patellar approach. The post-operative follow-up revealed successful healing and mobility improvement.
