Osteochondritis dissecans (OCD) of the elbow joint predominantly affects the capitellum, while OCD of the trochlea is relatively rare. A 19-year-old male badminton player presented with right elbow pain that persisted for 2 years. Imaging revealed trochlear osteochondral fragments. Owing to the large size and advanced stage of the lesion, osteochondral autologous transplantation (OAT) from femoral condyle was performed through the 10 mm diameter bone tunnel that was created in the olecranon via an open approach. The patient fully recovered and returned to competitive play 6-month postsurgery./r/nTransolecranon without olecranon osteotomy serves as a novel approach for OAT in the treatment of trochlear OCD.
