Isolated tarsal bone injuries and in particular involving either of 3 cuneiforms is an absolute rarity. A 63-year-old man presented with isolated middle cuneiform fracture with dorsal dislocation of the fractured fragment following a relatively common injury but resulting in a unique presentation due to forefoot positioning affecting the injury mechanism./r/nPrompt diagnosis with routine radiographs and definitive anatomical fixation can result in a pain-free outcome at 1-year follow-up. The paucity of literature and lack of definitive treatment guidelines made this a case worth sharing, for both its academic merits and treatment challenges.
