A 35-year-old patient developed iatrogenic medial patellar instability (IMPI) following arthroscopic capsular release for arthrofibrosis at an outside institution. Following primary lateral patellofemoral ligament reconstruction (LPFLR), the patient presented with recurrent superior and medial patellar instability after failure of her surgical construct during rehabilitation. A novel revision procedure was performed using a double-limbed semitendinosus allograft for LPFLR and iliotibial band autograft to reinforce the lateral patellotibial ligament./r/nThis report highlights the unique patient presentation of recurrent IMPI, a novel revision surgical technique, and biomechanical considerations associated with revision lateral patellofemoral complex reconstruction, adding to the limited literature on managing persistent IMPI.
